What information do I need to include in a letter of authorization (LOA)?
There are very few differences on requirements from the manufacturers. The LOA should be on company letterhead, it should include what product(s) they are ordering. Where the order should be shipped to. Your company name and your distributor name as being authorized to purchase the product from the manufacturer. It should be signed, and the signee should include his title and a phone number. You will want to check with your distributor to see if the manufacturer will accept a fax copy or if they require the original document.
